ACES Forms

 
Contract Change & Vacation Day Request Forms must be returned to the ACES billing office. ACES Sites will not accept them.

Contract Change Form
A 2 week notice is required for all changes. Contract Changes must be submitted in writing, per this form, to the ACES billing office or online per your ACES account. Verbal notification is NOT sufficient. 
 
Vacation Day Request Form
A 5 business day notice is required for all vacation requests. Vacation day requests must be submitted in writing, per this form, to the ACES billing office. Verbal notification is NOT sufficient. 
 
Medication Authorization Form
This form must be filled out and returned to the ACES site before your child's first day. The medication authorization form must be renewed each ACES session. Written parent permission and written physician orders are required before any medication is dispensed during the ACES program. Medications must be in their original container with proper label. Site Supervisors do not have access to the school nurse's office. Parents are responsible for transferring medication to the Non School Day/Summer Sites.
